The assessment involves meeting a person at home and asking questions to get a complete picture of their health and wellbeing. We can use this information to develop a health and wellbeing plan to keep people well and provide practical support when needed.
It is a 2 page written summary of the Holistic Aged Care Assessment with any actions and referrals that have been recommended.
Case management involves meeting a person at home on an ongoing basis to provide further assessment, monitoring and adjustment of any services arranged. It also allows for the health professional to provide further education and an opportunity to meet with family and other important people.
