This is the age at which the government has established an individual can access aged care services in Victoria, younger individuals will access different supports under disability services. That being said if you are of Aboriginal or Torres Strait Islander descent or have exhausted all options in the disability sector then we can still help!
  1. A completed new client registration form: Available on the homepage of this website or can be sent to you.
  2. A completed consent form: Available on the homepage of this website or can be sent to you.
  3. A list of current medications OR all the medication packets
  4. A list of previous health conditions.
Of course, however if they can’t make it don’t worry there will be a written report completed that family can refer to summarising the consultation and any recommendations.
No, unfortunately Aged Care Consultation services are not covered by private health insurers
No, unfortunately Medicare does not offer a rebate for aged care consultation services.
Don’t worry, we can complete the consultation over the phone or Skype, we just prefer to get to know you!
Fixed fee of $250
Netbank transfer, cash and credit / debit card are all accepted.
Clients have the option of:
  1. Initial consultation only $250: individualised education, advice and recommendations
  2. Aged Care Package $600: Initial consultation + 1 month of case management which includes: completion of any recommended referrals, unlimited telephone / email support / ongoing advice
PLEASE NOTE: External providers such as local councils or private home care providers who you may decide to engage with following the initial consultation usually have their own fees for services provided such as cleaning, shopping assistance etc.
