We specialise in the assessment and care of older people living at home.
Having some information prepared beforehand will mean we can make the most of the time during the home visit. Please bring:
  1. A completed new client registration form (see appendix 1) and consent form (see appendix 2) that we will send to you prior to your appointment.
  2. A list of current medications OR all the medication packets
  3. A list of previous health conditions.
Of course, however if they can’t make it don’t worry there will be a written report completed that family can refer to summarising the consultation and any recommendations.
Unfortunately, Holistic Aged Care Assessments and Case Management services are not covered by private health insurers.

Should you decide it would be helpful to have additional home based eg: cleaning assistance and / or health-based services eg: Physiotherapy they will have their own fees which will be discussed with you prior to commencing.

Absolutely, we believe this is the best place to understand a person and their needs. We visit people at home across Metropolitan Melbourne.
Yes, we can visit people at home during the evenings and on weekends if needed.
Of course, it is important we listen to you and important people in your life. However, if family can’t make it don’t worry there will be a written care plan completed that they can refer to summarising the consultation and any recommendations.
No problem, you are welcome to come to our office in South Kingsville 3015 (near Science works) or we can complete the Holistic Aged Care Assessment via video conference.
It involves meeting a person at home and asking questions to get a complete picture of their health and wellbeing. We can use this information to develop a health and wellbeing plan to keep people well and provide practical support when needed.
It is a 2 page written summary of the Holistic Aged Care Assessment with any actions and referrals that have been recommended.
Case management involves meeting a person at home on an ongoing basis to provide further assessment, monitoring and adjustment of any services arranged. It also allows for the health professional to provide further education and an opportunity to meet with family and other important people.
  • Allied health: Physiotherapy, Occupational Therapy
  • Mental health support: Psychology
  • Specialist nurses to optimise chronic health conditions such as diabetes
  • Specialist clinics: Falls clinic, Memory clinic, pain clinics.
  • Equipment: Walking aids, personal alarms.
* Occasionally a referral can only be completed by a family doctor.
  • Practical assistance in the home: Help with cleaning, shopping, meals, showering / dressing.
  • Help for carers: In home respite
  • Planning for the future: Arranging paperwork for respite in an aged care facility.
